1. Begin with the decision

Choose indicators that help a defined audience make a real decision. A measure without an intended use becomes reporting burden.

2. Write an operational definition

Specify numerator, denominator, inclusion and exclusion rules, data source, frequency, owner, direction of improvement, and interpretation.

3. Balance the measurement set

Use a small mix of outcome, process, and balancing measures so improvement in one area does not hide harm or burden elsewhere.

4. Make variation visible

Review trends over time with targets and meaningful signals. Avoid judging performance from a single isolated month.
